29 CFR1910.28(b)(1)(I):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by guardrail systems, safety net systems, or personal fall arrest, travel restraint, or positioning systems:    a)  On or about April 12, 2018, and at times prior, employees were exposed to fall hazards in excess of seven feet where an upper level storage area in the shipping and receiving department did not have guardrails in place.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use did not present a health hazard to the user:    a)  On or about April 12, 2018, and at times prior, the employer had not establish and implemented the elements of a written respiratory protection plan to ensure employees in the screen cleaning room voluntary using half-face respirators were medically able to use the respirators and that the respirators were cleaned, stored, and maintained so that the use did not present a health hazard to the user.
