29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (General Industry Reference 1910.28(b)(12)(i))  Employee rode on a job made scaffold platform on the forks of a rough terrain forklift at a height greater than ten feet and was not provided fall protection.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   An employee stood on job made plywood platform as it was lowered to the ground level, at a height greater than 10 feet, and the platform had not been adequately secured to the fork.
