1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) AMLI at Northwinds Apartment Clubhouse/Rental Offices, Roof: On or about 04/03/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that protective eyewear was not donned prior to operating these tools a 120 PSI. Employees were exposed to struck-by hazards which include roofing debris, metal collated fragments, and fasteners.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired: (a) AMLI at Northwinds Apartment Homes Clubhouse/Rental Offices, Front entrance walkway: On or about 04/03/2018, and at times prior, the employer exposed employees to fall hazards of up to 16-feet, in that the ladder provided had a damaged anti-slip safety shoe/foot and was not repaired or removed from service before employees were cleared to use the ladder to access the roof.
