29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 13, 2018, and at times prior thereto, one employee was not wearing head protection while performing construction work, exposing the employee to the hazard of being struck by falling materials such as pneumatic hammers, and building materials.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about April 13, 2018, and at times prior thereto, two employees were not protected by eye or face protection while using pneumatic hammers on the construction site, exposing the employees to the hazard of being struck by flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about April 13, 2018, and at times prior thereto, one employee was sitting on the top of a wall marking lines for the floor joist on the apartment complex under construction, exposing the employee to a fall hazard over 6 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about April 13, 2018, employees working on the apartment complex under construction had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazard of approximately 21 feet.
