29 CFR 1904.32(a)(4): At the end of each calendar year, the employer did not post the annual summary of injuries and illnesses from February 1 to April 30.      On or about April 13, 2018, the employer did not post the OSHA 300A (Summary) in which reported work-related injuries and illnesses for the calendar year of 2017. The OSHA 300A was not posted between February 1st and April 30th of the following year (2018).      There is no abatement certification or documentation required for this item.

29 CFR 1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  On or about, April 13, 2018, working space in front of both a 220 V and 480 V wall-mounted electrical panel on the north wall of building #2 was used for storage and access for ready and safe operation and maintenance was partially blocked by equipment and other materials.    There is no abatement certification or documentation required for this item.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   On or about April 13, 2018, an employee was exposed to skin and eye irritants while working with cleaning chemicals such as, but not limited to, bleach and the employer did not development a written hazard communication program.  There is no abatement certification or documentation required for this item.
