This condition exposed employees to amputation hazards.

This condition exposed employees to amputation hazards.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  On or about April 12, 2018 and at times prior, the employer did not certify that employees who operate forklifts had completed the practical portion of their forklift training. This condition exposed employees to the hazard of forklift accidents.
