29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a). Building 10 - On or about April 12, 2018, an employee operated a pneumatic nail gun without the use of eye protection, thus exposing the employee to a struck-by hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    a). Building 10 - On or about April 12, 2018, an employee did not have a lanyard attached from his body harness to the basket of a JLG Telescopic Boom Lift (model: 660SJ), thus exposing the employee to a fall hazard of approximately 22.2 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). Building 10 - On or about April 12, 2018, an employee installed wood siding from a third floor balcony without a fall protection system in place, thus exposing the employee to a fall hazard of approximately 22.2 feet.
