29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:  (a)  On or about April 10, 2018, and at times prior, employees were exposed to fall hazards of nine feet and greater in that they worked from the third level deck adjacent to the leading edge without fall protection in place.  (b) On or about April 10, 2018, and at times prior, employees were exposed to fall hazards in excess of six feet in that the open sided floor above the stairwell located on the west side of the building did not have a guardrail system in place.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening:  (a)  On or about April 10, 2018, and at times prior, employees were exposed to fall hazards of approximately twelve feet to ground level where a four foot section of the outside west wall was unprotected. 29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:  (a)  On or about April 10, 2018, and at times prior, workers were exposed to fall hazards at the second level elevator access where the guard rail system was not constructed to withstand a force of 200 pounds in the outward direction.
