29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) Jobsite, near 88 DuPont Street, Johnstown, PA: On April 12, 2018, a Project Planner was operating a rough terrain JLG forklift that came into contact with overhead, energized 23kv power lines that resulted in the electrocution of one employee and the hospitalization of two others due to electric shock.
