29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On 4/12/2018, employees from Heimburger Construction, Inc. were working on a commercial building located at 2420 Hopewell Road in Wentzville, MO. An employee was working from a JLG Aerial Lift, Model 460SJ, on the side of the building near the roof approximately sixteen feet above the ground. The employee was wearing personal fall arrest system, but the lanyard was not attached to the boom or basket.
