29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: On or about April 11, 2018, the employer did not implemented feasible administrative or engineering controls where employees are overexposed to noise level exceeding 90 decibels which would prevent hearing lost. (a). Shop: The Band Saw Operator was exposed to noise at 276.0% of the permissible daily exposure (8-hour time weighted average of 80 dBA) or the equivalent sound level of approximately 97.3 dBA during a 405 minutes sampling period on 4/11/18: exposure calculation includes a zero increment for the 75 minutes not sampled. General methods of control applicable in this circumstance include, but not limited to, the following: Rotate employees so that their exposure levels do not exceed those listed in Table G-16. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: On or about April 11, 2018, the employer did not maintain an audiometric testing program for employees who were overexposed to noise level exceeding 85 decibels which will help in determining hearing lost. (a). Shop: The Nail Gun Operator was exposed to noise at 101.9% of the permissible daily exposure (8-hour time weighted average of 80 dBA) or the equivalent sound level of approximately 90.1 dBA during a 242 minutes sampling period on 4/11/18: exposure calculation includes a zero increment for the 238 minutes not sampled. (b). Shop: The Band Saw Operator was exposed to noise at 289.0% of the permissible daily exposure (8-hour time weighted average of 80 dBA) or the equivalent sound level of approximately 97.7 dBA during a 405 minutes sampling period on 4/11/18: exposure calculation includes a zero increment for the 75 minutes not sampled. (c). Shop: The Band Saw Operator was exposed to noise at 117.8% of the permissible daily exposure (8-hour time weighted average of 80 dBA) or the equivalent sound level of approximately 91.1 dBA during a 406 minutes sampling period on 4/11/18: exposure calculation includes a zero increment for the 74 minutes not sampled. (d). Shop: The Band Saw Operator was exposed to noise at 144.9% of the permissible daily exposure (8-hour time weighted average of 90 dBA) or the equivalent sound level of approximately 92.7 dBA during a 398 minutes sampling period on 4/11/18: exposure calculation includes a zero increment for the 82 minutes not sampled. (e). Shop: The Band Saw Operator was exposed to noise at 94.4% of the permissible daily exposure (8-hour time weighted average of 90 dBA) or the equivalent sound level of approximately 89.6 dBA during a 425 minutes sampling period on 4/11/18: exposure calculation includes a zero increment for the 55 minutes not sampled. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
