29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: On or about April 13, 2018, the employees encountered a pipe that was not marked and cut through it assuming it was a water line but it was a gas line. The employer did not ensure that the exact location of the installations was determined by a safe and acceptable means. The employees were exposed to health and struck-by hazards.
