29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On 4/10/2018, employees from Schwada Builders, Inc. were installing insulation on the side of a commercial building located at 720 N. Service Road in Foristell, MO. Four employees were working from a Genie Aerial Lift, Model S-60 and JLG Aerial Lift, Model 400S. Three of the employees did not have their personal fall arrest systems attached to the boom or basket with a lanyard. The employer did not ensure that the jobsite was inspected by a competent person and the hazards were eliminated or controlled. Schwada Builders, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1140986, citation number 1, item number 1/a, issued on 7/15/2016, and was affirmed as a final order on 8/09/2016, with respect to a workplace located at 1992 Old Grace Road, Meredosia, IL.

On 4/10/2018, employees from Schwada Builders, Inc. were installing insulation on the side of a commercial building located at 720 N. Service Road in Foristell, MO.
