OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire, explosion, and deflagration hazards:     a) On or about 4/3/2018, In the facility:   Equipment such as, but not limited to cyclones, duct work, dust collectors and receiving containers associated with five Crucible Blaster Assemblies contained potentially explosive atmospheres during normal operation while handling combustible metals.  The employer did not ensure that the equipment was equipped and maintained with means of protection to prevent explosions, fires, and deflagrations.       Abatement documentation is required for this item.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a) On or about 4/17/2018, In the Cruciible Area:  A platform was not provided with a guardrail system or other means of fall protection.  Employees exposed to falls of 5.5 feet.      b) On or about 4/17/2018, In the Melt Area:  An opening at the ESR 2 furnace was not provided with guardrails or other means of fall protection.  Employees exposed to falls of 18 feet.          c) On or about 4/18/2018, In the Bar Mill:  There was lack of a midrail near the stairs to the Orbit saw platform.  Employees exposed to falls of 6 feet.     d) On or about 4/18/2018, In the Bar Mill:  The platform located behind Orbit Saw 1 was not provided with a midrail.  Employees exposed to falls of 8 feet.       Abatement certification is required for this item.

The platform was at the Wheelabrator 602 and was 8 feet above the floor, a chain rail was provided.    f) On or about 4/17/2018, In the Crucible Area:  A self closing gate was not provided for a platform accessed via a ladder.  The platform was 5.5 feet above the floor, no chain rail was provided.    g) On or about 4/18/2018, In the Bar Mill:  A self closing gate was not provided for a platform accessed via a ladder.  The platform was at the in line grit blaster and was 11 feet above the floor, a chain rail was provided.    Abatement certification is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)     On or about 4/17/2018, VAR Area:  The pistons on air compressor #1 were not guarded.  Employees exposed to moving parts.     b)     On or about 4/17/2018, VB3 area:  The rotating head on the VB3 system had handles that protruded from the frame.  Employees were exposed to being struck and caught by the handles.      c)     On or about 4/18/2018, In the Bar Mill:  Employees were exposed to a rotating ring support with damaged edges, along with pinch point hazards at supports.      d)    On or about 4/18/2018, In the Bar Mill:  Feed rollers on the BT-5 were not guarded.      e)    On or about 4/18/2018, In the Bar Mill:  The access to the Spiral Grinder was not adequately controlled.
