On or about April 12, 2018, at the jobsite located at Waterway Express Car Wash, 2261 South Kingshighway Boulevard Saint Louis, Missouri, 63110, employee(s) for GBI, Inc. were observed installing a parapet around, a 4:12 pitch, low-slope roof with unprotected sides and edges without any means of fall protection. The fall distance to the next lower level was approximately 20 feet.

The fall distance to the ground below was approximately 20 feet.
