On 3/19/18, at the Duluth job site, the employer did not designate pedestrian walkways in areas where employees are exposed to powered industrial truck traffic. 29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. On 3/19/18, at the Duluth facility, powered industrial truck cage attachments were used without the manufacturer's prior written approval so as to ensure the weight of the truck and attachment combination at maximum elevation with load was laterally centered. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
