29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   During roofing work at the 314 Pointe Loma Blvd., Lake St. Louis, residential site, employees were not wearing harnesses, nor was there any attachment to a fall life line.  (Note: safety belts may not be used in place of harnesses, and the personal fall arrest equipment must be attached to life lines during exposure to a fall.)

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  During roofing work at the 314 Pointe Loma Blvd., Lake St. Louis, residential site, the ladder used to access the roof did not extend three feet above the edge of the roof, but was less than 0.5 ft.
