29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. Live electrical parts were exposed due to a missing knockout approximately one inch in diameter. On or about April 17, 2018, on the west side of the Machine shop bay, the employer did not ensure that all unused breaker slot openings located inside of a breaker box were covered.
