Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5. Riverside Park Enterprises, Inc.; dba Six Flags New England, 1623 Main St., Agawam, Massachusetts; On and before April 11, 2018, electrically rated gloves used by the electricians were not retested or replaced at the 6 month interval required in Table 1.

29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): Six Flags New England, Gotham City Gauntlet, 1623 Main St., Agawam, Massachusetts; On and before April 11, 2018, while troubleshooting brake faults on the Gotham City Gauntlet roller coaster, employees were in the danger zone to visually verify the limit switch function with the roller coaster cars in motion.
