This violation occurred on or about April 9, 2018, SEI Millwrights did not conduct training for safely entering and exiting bins at the Sooner Coop in Hitchcock, Oklahoma exposing employees to the hazards of falls.

This violation occurred on or about April 9, 2018, SEI Millwrights did not provide training in rescue procedures for the employee standing outside the bin while another employee was in the bin cleaning out the old grain exposing employees to fall hazards.

29 CFR 1910.272(g)(2): Whenever an employee enters a grain storage structure from a level at or above the level of the stored grain or grain product, the employer did not equip the employee with a body harness with lifeline, or a boatswain's chair that meets the requirements of subpart D of this part. This violation occurred on or about April 9, 2018, SEI Millwrights did not ensure that the employee entering from the top of the bin was wearing a safety harness and adequate lifeline for retrieval from the bin exposing the employee to fall hazards.

29 CFR 1910.272(g)(4): The employer did not provide equipment for rescue operations which is specifically suited for the bin, silo, or tank being entered. This violation occurred on or about April 9, 2018, SEI Millwrights did not provide equipment for rescue operations for the employee who entered from the top of the bin and rappelled approximately 91 ft. to the bottom of the bin to clean out the old grain exposing employee to fall hazards.
