Rapid Reaction, Inc. - On or about April 18, 2018, and times prior thereto: Company employees worked with or around corrosive chemicals including, but not limited to, Purple Power Concentrated Industrial Strength Cleaner/Degreaser and battery acid.  Facilities for quick drenching or flushing of the eyes were not available to employees.

Rapid Reaction, Inc. - On or about April 18, 2018, and at times prior thereto: employees worked with or around hazardous substances including, but not limited to, degreasers, oils, lubricants, battery acid, and carbon monoxide. The employer had not developed and implemented a written Hazard Communication Program.

29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:   a.) Rapid Reaction, Inc. - On or about April 18, 2018, containers of hazardous chemicals including, but not limited to, Rust Lick 631, Purple Power Concentrated Industrial Strength Cleaner/Degreaser, and Mr. Clean, located throughout the production area, were not labeled with the hazards of the hazardous chemical(s)contained therein.

Rapid Reaction, Inc. - On or about April 18, 2018, and at times prior thereto: employees worked with or around hazardous substances including, but not limited to, degreasers, oils, lubricants, and carbon monoxide. Safety data sheets for hazardous chemicals including, but not limited to, Rust Lick 631, Trim E206, and Waylube 68 were not maintained at the worksite nor was there a method/procedure in place to ensure SDSs could be provided to an employee requesting access to the document.
