29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: On or about April 18, 2018, in the vehicle repair shop bay, the employer did not ensure that combustible gas cylinders were stored in a safe manner to prevent injury. An oxygen cylinder and acetylene cylinder were stored together without a noncombustible barrier or being separated a minimum of 20 feet.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). A:) On or about April 18, 2018, at the East Shop(stainless pipe fab shop), the employer allowed employees to use a motorized electric pipe roller with power cords that had slag burns and was covered with electrical tape. B:) On or about April 18, 2018, at the Main shop A bay bender machine, the employer allowed employees to use a cold pipe bender that had power wires with spliced wire caps and exposed wires.
