29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  a) Trim Saw: On May 18, 2018 and at time prior thereto the employer had not established a formal housekeeping program.  Over an estimated two day period, combustible wood dust and other debris had accumulated under and around the trim saw machine that was several inches deep.      Note) Certification of corrective action is required for this item.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) Building 1 and 2: On April 18, 2018 and a t times prior thereto, clear, easy access to portable fire extinguishers was not well maintained.  Stacks of pallets and other items were stored near fire extinguishers. Extra or discarded band saw blades were hung around a fire extinguisher.  Signage was not positioned to easily identify the location of the portable fire extinguishers.      Note) Certification of corrective action is required for this item.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative:  a) Facility: On April 18, 2018 and at times prior thereto employees performed servicing and unscheduled maintenance on machines including but not limited to the notcher machine.  A formal energy control program with procedures, training, and periodic review had not been developed to provide authorized employees with specific procedures and affected employees with information on their roles and responsibilities within an energy control program.
