(a) Kitchen roof:  On or about April 11, 2018, and times prior, as part of a fall protection system, while working at heights of up to 18 foot, an employee used a 50ft. vertical lifeline and rope grab connected to a 3ft. rope positioning lanyard with snaphooks at each end that was visibly rusted and corroded.
