29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l), and as referenced by Construction Standard 1926.602(d): (a) Chapmanville Intermediate School jobsite: On or about April 12, 2018 and times prior, the employer did not ensure the operator of the JLG Skytrak Telehandler had been trained and evaluated prior to operating the lift on the jobsite.
