29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about April 18, 23870 Flynt Drive, Daphne, Al: The employer failed to ensure an employee installing sheathing on a residential home used fall protection which exposed them to fall hazards from a 8:12 pitch roof of 9'3" to the ground below.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  a) On or about April 18, 2018 at 23870 Flynt Drive, Daphne, Al: The employer failed to ensure an employee properly used his fall protection in that he wrapped the rope around a truss and connected the snap hook back to the rope which exposed him to fall hazards.
