29 CFR 1910.29(f)(4): Openings in a stair rail system exceeded 19 inches at its least dimension: a) Building 1609 County, Production shop: On or about 04/12/2018, the fixed flight of steps that led to the mezzanine, a mid-rail was not installed leaving 28-31 inch openings between the top rail and step thus exposed employees to an 8-foot fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): a) Building 1609 County, Production shop: On or about 04/12/2018, employees welded on A36 metal that according to the safety datasheet contained approximately 14 percent chromium, the employee's 8-hour time-weight average exposure was not determined therefore employees were potentially overexposed to toxic fumes. The employer is to make this determination in accordance with either paragraph (d)(2), the scheduled monitoring option, or paragraph (d)(3), the performance-oriented option. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
