29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through the holes, by covers:  (a) On or about April 18, 2018, employees were exposed to falls through floor holes that were uncovered. The holes were on the first floor behind stairways, and were available to access piping.  A cover was adjacent to one of the holes and was not in use.
