29 CFR 1926.302(b)(3): All pneumatically driven nailers, staplers, and other similar equipment provided with automatic fastener feed, which operate at more than 100 p.s.i. pressure at the tool shall have a safety device on the muzzle to prevent the tool from ejecting fasteners, unless the muzzle is in contact with the work surface.  (a)  Second floor hallway - On or about May 18, 2018 employees were using a Hitachi NR 38 nailer that had the Safety device on the muzzle of pneumatically driven nailers, staplers, and other similar equipment did not prevent the tool from ejecting fasteners as required.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  (a) Along rear side of house - On or about May 18, 2018 employees framing the new house were walking along the wooden framed scaffolding planking that was not 18 inches wide.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Rear side of house on second floor - On or about May 18, 2018 employees framing a new house were walking along the open rafters that is 9 feet above the lower landing without any fall protection.
