29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:     On or about April 18, 2018, during roofing operations being performed at a residential worksite located at 8872 Scotsbury Glen St NW, Massillon, Ohio 44646, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of heights of approximately 14.4 feet above the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about April 18, 2018, during the installation of roofing material at a job site located at 8872 Scotsbury Glen St NW, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 14.4 feet by utilizing a fall protection system.       Shrock Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1147120, citation number 01, item number 02 and was affirmed as a final order on 10/13/2016, with respect to a workplace located at 9600 Mt. Clair Blvd NW, Massillon, Ohio 44646.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about April 18, 2018 at the job site located at 8872 Scotsbury St NW, Massillon, Ohio 44646; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.       Shrock Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1147120, citation number 01, item number 03 and was affirmed as a final order on 10/13/2016, with respect to a workplace located at 9600 Mt. Clair Blvd NW, Massillon, Ohio 44646.
