29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Kitchen: On April 18, 2018 and times prior, the employer did not provide machine guarding for a 140 quart Hobart Vertical Mixer (model #V-1401, serial #1935066), to protect employees from the rotating mixer attachments, such as flat beater, dough hook and wire whip. (b) Kitchen: On April 18, 2018 and times prior, the employer did not provide machine guarding for an 80 quart Hobart Vertical Mixer (model #L-800, serial #1327329), to protect employees from the rotating mixer attachments, such as flat beater, dough hook and wire whip. (c) Kitchen: On April 18, 2018 and times prior, the employer did not provide machine guarding for a 60 quart Hobart Vertical Mixer (model #H-600, serial #11182398), to protect employees from the rotating mixer attachments, such as flat beater, dough hook and wire whip.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) Back Kitchen Wall: On April 18, 2018 and times prior to, the area in front a two electrical panels that contain the electrical disconnects for the ovens, fryers, lighting and outlets was used for storage of bakery equipment.
