29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) do not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): a) Brake Cable Area- On or about April 12, 2018, the employer did not ensure that all point of operation guards on machinery, such as the Federal Press Punch Presses #1, #3, and #4 were designed to prevent employees from having their hands or fingers in the danger zone. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.217(c)(2)(vi): The adjustable barrier guard(s) on mechanical power press(es) were not securely attached to the press bed, bolster plate, or die shoe in a manner that would permit the adjustments to be made by authorized personnel whose qualifications included a knowledge of the provisions of Table 10 O-10 and 29 CFR 1910.217(c): a) Brake Cable Area- On or about April 12, 2018, the employer did not ensure that adjustable sliding guards on machines, such as the Federal Punch Press #2, was affixed and/or securely attached in a manner that would prevent employees from having any part of their body in the danger zone during the operating cycle. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
