29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Chemical storage area:  On or about April 17, 2018, the employer did not provide suitable facilities for quick drenching or flushing of the eyes to protect employees from chemicals such as but not limited to glycol and ammonia.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks:  Labeling area:  On or about April 17, 2018, employees were allowed to circumvent the safety guard on a Trine labeling system by lifting the safety guard and reaching into the system to change out the labels with the system running.
