On or about April 19, 2018, at the above addressed inspection site, a harness with lanyard attached to the boom or basket was not worn by employee(s) operating an aerial lift. Employees were thereby exposed to a fall of approximately 6 feet. There is no abatement certification of documentation required for this item.
