29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.      In the Fabrication Department, the portable belt conveyor used by employees by the die-press machine was not guarded exposing employees to amputation or caught-in hazards.       The Packaging Corporation of America was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1194296, citation number 1, item number 1 and was affirmed as a final order on April 3, 2017, with a final abatement date of April 3, 2017, with respect to a workplace located at 2111 Hester Avenue, Donna, TX 78537.    The Packaging Corporation of America was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 1910.212(a)(1), which was contained in OSHA Inspection number 1279026, citation number 1, item number 1 and was affirmed as a final order on January 30, 2018, with a final abatement date of February 20, 2018, with respect to a workplace located at 708 Killian Road, Akron, OH 44319.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet).  The employer had four doors enroute to the maintenance department that were not identified with a sign, indicating its purpose, exposing employees to entrapment hazards.
