29 CFR 1910.178(l)(4)(iii) as required by 1960.8(b): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a. On or about April 13, 2018, employee who was required to operate the CAT Forklift, Model # 2P4000, S/N AT35532082 to move a lawn mower was not evaluated for operator performance at least once every three years. b. On or about April 6, 2018, employee who was required to operate the CAT Forklift, Model # 2P4000, S/N AT35532082 to move cardboard bales was not evaluated for operator performance at least once every three years. The Army & Air Force Exchange Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 1203230, notice number 1, item number 2 and was affirmed as a final order on February 22, 2017, with respect to a workplace located at 5443 Dudley Blvd., Bldg. 911, McClellan, CA 95652.
