29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) Packaging area - Graphic Packer The employer did not guard belt and pulley on Graphic Packer (serial# QF6-114). On 4/10/18, an employee place his hand in the out feed side of machine to unjam case that was stuck . The machine was started and the employee caught his right pinky finger between belt and pulley causing de-gloving which then required partial amputation of finger. Occurred on or about 4/10/2018
