29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) Throughout facility    The employer did not develop and document equipment specific procedures to be utilized for the control of hazardous energy for employees who perform repair and maintenance on equipment including but not limited to,  Graphic Packer and Cooling Tunnel. 29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    a) Packaging area - Graphic Packer    The employer did not guard belt and pulley on Graphic Packer (serial# QF6-114).  On 4/10/18, an employee place his hand in the out feed side of machine to unjam case that was stuck .  The machine was started and the employee caught his right pinky finger between belt and pulley causing de-gloving which then required partial amputation of finger.    Occurred on or about 4/10/2018

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven -7 feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:    a) Packaging area    The employer did not guard horizontal belt on Graphic Packer (serial# QF6-114).  On 4/10/18, an employee place his hand in the out feed side of machine to unjam case that was stuck . 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) 45 Camptown Rd., Maplewoood, NJ    The employer did not conduct an annual evaluation of energy control procedures in 2017 to ensure the compliance with the requirements of the standard to correct any deviations or inadequacies identified.    Condition observed on or about 4/25/2018    Maplewood Beverage Packers LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147 (c)(6)(i), which was contained in OSHA inspection number 951673, citation number 1, item number 10 and was affirmed as a final order on 12/01/14, with respect to a workplace located at 45 Camptown Rd., Maplewood, NJ.
