29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than 1 inch wide except where the employer can demonstrate that a wider space is necessary: Chinle Elderly Care Renovation, #170298: The employer did not ensure that a supported scaffold used for stucco application was fully planked between the platform and the uprights creating a slip, trip, or fall hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: Chinle Elderly Care Renovation, #170298: The employer did not ensure that a base plate of a supported scaffold used for stucco application was completely supported by the mud sill which can cause it to shift and move. Additionally, a piece of scrap particle board was used as the mud sill.
