Instance b. Site - On or about April 2, 2018, employees engaged in framing activities were exposed to unguarded or unprotected floor openings where falls exceeded 30 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Instance a. Site - On or about April 2, 2018, employees accessing the third floor work area through the floor openings were not protected from falls of approximately 10 feet where three Werner 8 foot stepladders were used to access an upper floor and each ladder did not extend 3 feet above that surface. Instance b. Site - On or about April 2, 2018, the Werner ladder in use by employees to access the third floor work area did not extend 3 feet above the upper landing surface.
