29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Quantico Marine Base, West End, Academic Instruction Facility Building: On or about 04/18/2018, employees used a 12-foot A-frame ladder that could not be extended 3 feet above the upper landing of an internal structure, as a result employees were exposed to a possible 15 feet fall hazard. No abatement certification or documentation required.
