29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)    Academic Instruction Facility Building, West End: On or about 04/17/2018, employees were on a roof installing trusses and were not protected from a possible 30 feet fall hazard.  No abatement certification or documentation required.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)    Quantico Marine Base, West End, Academic Instruction Facility Building: On or about 04/18/2018, employees used a 12-foot A-frame ladder that could not be extended 3 feet above the upper landing of an internal structure, as a result employees were exposed to a possible 15 feet fall hazard.    No abatement certification or documentation required.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a)    Quantico Marine Base, Academic Instruction Facility Building: On or about 04/18/2018, employees used a 12-foot A-frame ladder to gain accesses to the top of an internal structure by standing on the top of the ladder.   No abatement certification or documentation required.
