29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) On or about December 8, 2017, employees were exposed to flying objects while not using appropriate personal protective equipment necessary to protect them from the hazard.  Employees were observed using pneumatic nail guns without appropriate impact-resistant safety glasses while securing roofing materials. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a)  On or about December 8, 2017, employees were exposed to fall hazards due to lack of a guardrail system, safety net system, or personal fall arrest system while engaged in residential construction activities more than 10 feet above a lower level.  Employees were observed walking into and standing in the bucket attachment on a forklift to retrieve materials while installing roofing materials at heights greater than 10 feet. Employees were observed installing roofing materials on a second story roof of a multi-family residence while wearing fall harnesses that were not attached to a lanyard or anchor. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about December 8, 2017, and at times prior, employees were exposed to fall from height hazards due to portable ladder side rails not extending at least 3 feet above the upper landing surface.  Employees used a portable ladder to access a second story roof.  At the time of inspection the ladder side rails extended approximately 12" above the upper landing surface.
