29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: Instance 1: On or about April 17, 2018, at the Tru by Hilton located at 1220 West Prien Lake Road in Lake Charles, LA, the employer did not receive written approval from Caterpillar prior to attaching a wooden man basket to a Caterpillar TL1255C Telehandler and using it to raise workers. Instance 2: On or about April 17, 2018, at the Tru by Hilton located at 1220 West Prien Lake Road in Lake Charles, LA, the employer did not receive written approval from Caterpillar prior to attaching a metal man basket to a Caterpillar TL1255C Telehandler and using it to raise workers.
