29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: Electrical cable was suspended +/- 25 feet over parking lot where truck traffic was present.  a) Front Parking Lot: Employees were exposed to scaffold collapse from an electrical cable that was suspended over a parking lot where truck traffic was present. An incident occurred where the cable was caught by the bed of a dump truck, causing a scaffold to collapse injuring an employee, on or about 4-23-18.
