29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) 123 E. Gramby Street, Manheim PA - An employee was observed using a JLG aerial lift to access the roof of the building and did not attach the lanyard of his personal fall arrest system to the boom or basket and was exposed to fall hazards up to approximately 26 feet to the ground below, on or about April 23, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Gramby Street, Manheim PA - Employees were conducting framing operations on a flat roof and were not protected from an approximate 26 foot fall hazard to the ground below, on or about April 23, 2018.
