29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system: Construction Site: On or about April 24, 2018; five employees were observed working (mounting antennas) on the roof of a two story school building without utilizing any type of fall protection equipment. This condition exposed the employees to a fall hazard greater than 6-feet to the ground below.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. Construction Site: On or about April 24, 2018, the employer did not ensure that the employees retained the requisite understanding to recognize the hazards of falling. This condition exposed employees to a fall hazard of more than 6-feet to the ground below.
