29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: On or about February 9, 2018, the employer directed employees to enter the large outdoor trash compactor to search for a hazardous material bag that was mistakenly discarded in the compactor without locking it out, exposing employees to the hazards created by unexpected reenergization of energy sources.
