29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 12, 2018, and times prior to, at Line 5 Batter Mixer Pump, an employee cleaning the batter mixing pump was exposed to an amputation hazard while the mixer was still plugged to a power source. Moving pump blades caused a partial amputation of the employee's finger. The employer failed to develop, document and utilize a specific lockout/tagout procedure for servicing, washout, and maintenance of the mixer pump. Employees walk and work in the area. or in the alternative: 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about April 12, 2018, and times prior to, at Line 5 Batter Mixer Pump, an employee was exposed to amputation hazards while cleaning the batter mixing pump that was still plugged to a power sources. Moving pump blades caused a partial amputation of the employee's finger. The employer failed to ensure adequate guarding of the pump during cleaning operations. Employees walk and work in the area.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about April 12, 2018, and times prior to, at the Line 5 Batter Mixer Pump, employees were exposed to an amputation hazard while cleaning a batter pump that was still running. Moving pump blades caused a partial amputation of the employee's finger. The employer failed to adequately train an authorized employee on the hazards associated with cleaning the batter pump. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
