29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Production area: On or about April 17, 2018, the employer exposed employees to amputation hazards in that, employees were not provided with lockout tagout training when performing operating and maintenance on equipment such as, but not limited to: the shankland, rollup and pad machines.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) On or about April 17, 2018, the employer exposed employees to chemical hazards in that, employees were using hazardous chemicals such as liquid propane gas and bleach and a hazard communication program had not been developed and implemented which included training.
