On or about April 25, 2018, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about April 25, 2018, at the above addressed jobsite, employees performing residential roofing operations on a roof, twenty (20) feet  above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
