29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems:  a. On or about April 24, 2018, at the above addressed jobsite, employees performing roofing operations on the roof, eighteen (18) feet five (5) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  Cayetano Guaman was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1276680, citation number 1, item number 1 and was affirmed as a final order on March 19, 2018, with respect to a workplace located at 7960 Cascade Court, Orland Park, IL 60462.

29 CFR 1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record.  a.  On or about April 24, 2018, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.   a.  On or about April 24, 2018, at the above addressed jobsite, employees had been provided fall protection training however, the employer failed to keep a written certification of record.
